Visual Loss with External Ophthalmoplegia after Shoulder Operation

Abstract
Postoperative visual loss rarely develops, but its outcome is usually unfavorable. The patho-mechanism is also uncertain, though embolism, hypoperfusion, venous congestion, and direct pressure to eye have been suggested. We present a case of post-operative unilateral visual loss and external ophthalmoplegia after shoulder surgery who showed ischemic optic neuropathy, central retinal artery occlusion, orbital myositis, and trigeminal nerve injury. Ischemic injury in the ophthalmic artery territory was a suspected patho-mechanism of this case. Visual loss was irreversible despite immediate treatments.
